Job Summary
Under minimal supervision, the Licensed Behavioral Health Care Coordinator (BHCC) is responsible for facilitating and coordinating the care delivered to an assigned group of members who are identified as having primary behavioral health needs (mental health and/or substance use disorder concerns) using multidisciplinary and member/family collaboration to ensure quality and cost effective outcomes are delivered within appropriate care coordination parameters. Coordination involves assessment of care needs, planning, support, and evaluation of member care and related outcomes. Activities to be performed are screening and assessment of medical, behavioral health and social determinants needs and gaps in care, collaboration with the Member to develop a care plan with SMART goals, scheduled outreach to support member in achieving their goals and supporting the Members self-efficacy to navigate systems. BHCC communicates closely with the Member's care team inclusive of: the Member, designated health representatives, primary care provider, behavioral health providers and other care coordinators involved with the Member's care. Encounters occur face to face, telephonically, and electronically with members in clinic and in other community-based settings.
Position is an active contributor in the development of systems (e.g. workflows, clinical pathways, assessments) to improve the care of assigned member populations. Helps ensure compliance with state, federal and third-party payer requirements.
Essential Functions:
In Members with primary complex behavioral health needs, use DHMP Care Management protocols and critical thinking to assess, identify opportunities to improve Member healthcare outcomes, collaboratively set SMART goals with the Member, develop a care plan with interventions and support and enable the Member to achieve their goals and independently navigate needed services to improve overall health outcomes. (15%)
Visits members with complex behavioral health needs in community based settings, including community centers, hospitals or providers' offices. (15%)
Provides a complete continuum of quality care through close communication with members via in-person, phone or electronic interaction (15%)
Supports members with education relevant to their disease process, medication reviews and connections to community resources such as food, housing or financial support programs. (10%)
The BHCC will work with members with complex needs, prioritize care management and work with a multidisciplinary team to support the needs of the Members on their caseloads (10%)
Documents in readable, understandable language according to professional, regulatory, and agency standards. (10%)
Documents and disseminates results of care to member, caregivers, and others involved in the care or situation, as appropriate, in accordance with contractual requirements, state and federal laws, regulatory requirements, and Denver Health policy. (10%)
Uses therapeutic approaches to identify the Member?s readiness for and ability to change behavior to support achieving Member?s goals. *Identifies and executes evidence-based interventions to support the Member in achieving their health goal (10%)
BHCC-DSNP will be responsible for a defined caseload of Members enrolled in the DHMP Medicare Choice Dually-eligible Special Needs Program (DSNP) who are identified as having a high risk acuity level and a primary behavioral health condition. *The DSNP Model of Care is a well-defined model requiring a comprehensive health risk assessment (HRA), individualized care plan (ICP) and integrated care team (ICT) review of the ICP. This routine is completed at least annually and more frequently when the Member experiences a change in condition, as evidenced by hospitalization, movement to a residential place of care or other event. *BHCC obtains Member consent to participate in the program as well as to share clinical information with the Member?s care team. *BHCC works in collaboration with the DSNP team and supports Members of the team including social workers, RNs and care coordinators with behavioral health expertise and an understanding of trauma-informed care approaches. Education:
Master's Degree In a mental health-related field such as psychology, counseling psychology, marriage or family therapy, or social work required
Work Experience:
1-3 years of direct care experience in providing care coordination, therapy, counseling, or other behavioral health setting required
Licenses:
LCSW-Licensed Clinical Social Worker - LCSW State Licensing Board requiredOR
LPC-Licensed Professional Counselor - LPC State Licensing Board required
Knowledge, Skills and Abilities:
Bilingual in English/Spanish preferred.
Must have excellent interpersonal skills and customer service skills.
Excellent written and oral communication skills.
Experience with trauma-informed care and ability to apply to individual Members and populations.
Knowledge of crisis intervention and ability to respond to trauma in both children and adults.
Knowledge of patient populations? cultural, religious, ethnic, and social systems in interactions and care planning.
Familiarity with community resources available to adults and families.
Ability to maintain firm boundaries and confidentiality among colleagues and Members
Ability to respond to feedback and incorporate into practice.
Comfort in providing group training and working collaboratively with others.1. Experience with windows-based computer programs and ability to use computer for data analysis and data display, required.
Experience with Medical Management platforms used to document care coordination services, preferred.
